ABSTRACT

Objective:To investigate the treatment efficacy of adjuvant anti-VEGF/VEGFR targeted therapy in patients with non-metastatic (cM 0) non-clear cell renal cell carcinoma and tumor thrombus (nccRCC-VTT). Methods:This retrospective study enrolled 26 patients who underwent radical nephrectomy combined with inferior vena cava tumor thrombectomy at Peking University Third Hospital from January 2014 to July 2021. Patients were divided into adjuvant therapy group (10 cases) and control group (16 cases)based on the use of postoperative targeted therapy. The distribution of baseline clinical characteristics in the adjuvant therapy group and the control group were as follows: gender (6 males and 4 females in the adjuvant therapy group, 12 males and 4 females in the control group, P=0.66), age (56.2±18.5 years old in the adjuvant therapy group; 54.6±14.5 years old in the control group; P=0.80), BMI(24.0±3.5 in the adjuvant therapy group; 24.3±3.3 in the control group; P=0.80), presence of clinical symptoms (8 cases in the adjuvant therapy group; 15 cases in the control group; P=0.54), tumor laterality(6 cases on the left and 4 cases on the right in the adjuvant therapy group; 6 cases on the left and 10 cases on the right in the control group; P=0.42), location of tumor thrombus (2 cases with renal vein tumor thrombus and 8 cases with inferior vena cava tumor thrombus in the adjuvant therapy group; 2 cases with renal vein tumor thrombus and 14 cases with inferior vena cava tumor thrombus in the control group; P=0.67), ASA classification (2 cases in ASA class 1 and 8 cases in ASA class 2 in the adjuvant therapy group; 2 cases in ASA class 1 and 14 cases in ASA class 2 in the control group; P=0.63), surgical approach (7 minimally invasive surgeries and 3 open surgeries in the adjuvant therapy group; 9 minimally invasive surgeries and 7 open surgeries in the control group; P=0.68), conversion to open surgery (2 cases in the adjuvant therapy group; 2 cases in the control group; P=0.63), operation time [287.5(222.2, 456.0) minutes in the adjuvant therapy group; 344.0(287.8, 482.5) minutes in the control group; P=0.34), blood loss [400.0(250.0, 600.0)ml in the adjuvant therapy group; 575.0(175.0, 800.0)ml in the control group; P=0.63), Clavien-Dindo classification of postoperative complications (8 cases with no postoperative complications, 2 cases with level 1-2 complications, and 0 cases with level ≥3 complications in the adjuvant therapy group; 10 cases with no postoperative complications, 4 cases with level 1-2 complications, and 2 cases with level ≥3 complications in the control group; P=0.68), postoperative hospital stay (8.5 [5.5, 11.5] days in the adjuvant therapy group; 7.5 [6.0, 13.0] days in the control group; P=1.00), maximum tumor diameter[ (9.2±2.7)cm in the adjuvant therapy group; (8.9±3.3)cm in the control group; P=0.81], sarcomatoid differentiation (0 cases in the adjuvant therapy group; 1 case in the control group; P=1.00), perinephric fat invasion (2 cases in the adjuvant therapy group; 7 cases in the control group; P=0.40), tumor necrosis (6 cases in the adjuvant therapy group; 5 cases in the control group; P=0.23), pathological subtype (1 case of PRCC type 1, 6 cases of PRCC type 2, and 3 cases of TFE3 rearrangement RCC in the adjuvant therapy group; 2 cases of PRCC type 1, 10 cases of PRCC type 2, and 1 case each of oncocytic PRCC, TFE3 rearrangement RCC, FH-deficient RCC, and unclassified RCC in the control group; P=0.72), WHO/ISUP nuclear grade (10 cases of grades 3-4 in the adjuvant therapy group; 4 cases of grades 1-2 and 12 cases of grades 3-4 in the control group; P=0.14), invasion of tumor thrombus into the vessel wall (5 cases in the adjuvant therapy group; 5 cases in the control group; P=0.43), T stage (1 case of T 3a, 3 cases of T 3b, 5 cases of T 3c, and 1 case of T 4 in the adjuvant therapy group; 1 case of T 3a, 4 cases of T 3b, 10 cases of T 3c, and 1 case of T 4 in the control group; P=1.00), and positive lymph nodes metastasis(3 cases in the adjuvant therapy group; 0 cases in the control group; P<0.05). The recommended doses for sunitinib, axitinib, and pazopanib are 50mg qd, 5mg q12h, and 800mg qd, respectively. The primary endpoint of this study was disease-free survival (DFS), and the secondary endpoint was overall survival (OS). Statistical analyses were performed using R v4.2.2. Confounding factors were adjusted using propensity score weighting. Results:The median follow-up time for DFS was 29 months in the adjuvant therapy group and not reached in the control group, while median follow-up time for OS was 28 and 26 months, respectively. In the univariate Cox regression analysis, there were no statistically significant difference in the impact of all baseline characteristics and exposure factors on DFS and OS between the two groups. In survival analysis, there were no significant difference between DFS and OS curves of patients in the adjuvant therapy group and the control group (DFS, P=0.62; OS, P=0.74). The median DFS of patients in the adjuvant therapy group and the control group were 17 and 19 months, respectively, while the median OS was 43 and 27 months. After adjusting for confounding factors, the median DFS of patients in the adjuvant therapy group and the control group were 26 and 12 months, respectively, and the median OS remained 43 and 27 months, with no significant difference (DFS, P=0.81; OS, P=0.40). Conclusion:There is currently a lack of definitive evidence for survival benefit from adjuvant anti-VEGF/VEGFR targeted therapy in patients with cM0 nccRCC-VTT after surgery.

ABSTRACT

Objective:To observe the effect of sagittal traction on the internal fixation with proximal femoral nail anti-rotation (PFNA) for femoral intertrochanteric fractures in the elderly patients.Methods:A retrospective study was conducted to analyze the clinical data of 61 elderly patients with intertrochanteric fracture who had been treated from January 2016 to January 2018 at Department of Orthopaedics, The Ninth Hospital of Ningbo. Of them, horizontal traction of the affected limb was performed in 31 (16 males and 15 females with an age of 73.6 years ± 9.4 years) while 10°-20° sagittal traction of the affected limb in 30 (15 males and 15 females with an age of 75.0 years ± 7.2 years). The 2 groups were compared in terms of operation time, intraoperative blood loss, times of intraoperative fluoroscopy, fracture healing time, hip function at 6 months after operation by Harris scoring and complications.Results:There was no statistically significant difference between the 2 groups in the general clinical data, showing comparability ( P>0.05). The operation time (47.1 min ± 4.7 min), intraoperative blood loss (72.3 mL ± 9.1 mL), and times of intraoperative fluoroscopy (11.5 ± 1.4) in the sagittal traction group were significantly less than those in the horizontal traction group (56.7 min ± 4.6 min, 103.9 mL ± 19.2 mL, and 14.6±1.6) ( P< 0.05). This cohort was followed up for 6 to 18 months (12.6 months on average). There were no significant differences between the 2 groups in quality of postoperative fracture reduction, fracture healing time or hip Harris scores at postoperative 6 months ( P>0.05). Internal fixation loosening and hip pain occurred in one patient in the sagittal traction group. Follow-ups observed no complications like wound infection, bedsores, deep vein thrombosis or hip varus deformity in either group. Conclusion:As 10°-20° sagittal traction can lift the affected limb upward to avid blockage by the iliac wing, hip soft tissue and lateral abdominal wall, it lends convenience to surgeons, reducing intraoperative bleeding volume, times of fluoroscopy and operation time.

ABSTRACT

OBJECTIVE: To study the effect of nano-cerium oxide on the early development of zebrafish embryos. METHODS: The well-developed zebrafish embryos were randomly divided into the control group and the 50, 100, 200, 400 and 800 mg/L dose groups, with 40 embryos in each group. The dose groups were treated with nano-cerium oxide at the corresponding mass concentration for 5 days. The control group received no treatment. The death and malformation of embryos were observed. The heart rate of zebrafish embryos was recorded using confocol microscope. The protein expression of microtubule-associated protein 1 light chain 3(LC3) and cleaved Caspase-3 and were observed by Western blot technology. RESULTS: The death and embryonic malformation rate of zebrafish embryos increased with the increase of doses, showing statistical significance(P<0.01). The heart rate of the 800 mg/L dose group was decreased compared with the control group [(77±8) vs(93±4) beats/min, P<0.01]. There was no statistical significant difference in LC3-Ⅱ protein expression in each groups(P>0.05). The cleaved Caspase-3 protein expression increased in all dose groups compared with the control group(P<0.05). The cleaved Caspase-3 protein expression in the 200 mg/L dose group was higher than that in the 50 mg/L dose group(P<0.05). CONCLUSION: The nano-cerium oxide may induce cell apoptosis, causing toxic effect in early development of zebrafish embryos.

ABSTRACT

This study was designed to determine the metabolites of Zhali Nusi Prescription(ZLNSP) in rats. The ultra-high performance liquid chromatography-LTQ Orbitrap mass spectrometric(UHPLC-LTQ-Orbitrap-MS) and mass defect filter techniques were applied to analyze the metabolites of ZLNSP in rat plasma, bile, urine and feces. The biological samples were analyzed by ACQUITY UPLC BEH T_3 column(2.1 mm×100 mm,1.7 μm), with 0.1% formic acid water(A)-acetonitrile(B) as mobile phase, and the biological samples were analyzed in negative ion mode by electrospray ionization mass spectrometry(ESI-MS). An analytical method for biological samples of rats was established, and 8 prototype components and 36 metabolites were identified. The results showed that the metabolic pathways of the main components of ZLNSP in rats included methylation, glucuronidation, sulfation and so on. It provi-ded information for the therapeutic effect of ZLNSP in vivo.

ABSTRACT

Objective@#To observe interleukin-6 (IL-6), interleukin-17 (IL-17), and tumor necrosis factor-α (TNF-α) levels in acute and chronic brucellosis before and after antibiotics treatment in order to assess the bone resorption.@*Methods@#Using group design, 36 acute and 36 chronic brucellosis before antibiotics treatment, and 36 acute and 36 chronic brucellosis after antibiotics treatment from the Endemic Disease Prevention and Control Center of Ulanqab City were selected as the research subjects, with 25 local healthy persons as the healthy control group from January to April 2017. Enzyme linked immunosorbent assay (ELISA) method was used to determine the serum levels of IL-6, IL-17, and TNF-α in acute brucellosis, chronic brucellosis and control groups before and after antibiotics treatment.@*Results@#The means of IL-6 [(23.19 ± 2.51), (21.93 ± 2.27), (14.52 ± 2.08) ng/L], IL-17 [(23.55 ± 2.24), (21.43 ± 2.44), (13.69 ± 1.85) ng/L], and TNF-α [(478.97 ± 47.11), (434.67 ± 42.11), (290.19 ± 34.67) ng/L] were significantly different in the acute brucellosis, chronic brucellosis and control groups before antibiotics treatment (F= 113.91, 152.88, 153.49, P < 0.01); compared to control group, the levels of IL-6, IL-17, and TNF-α in acute brucellosis and chronic brucellosis groups were significantly increased before antibiotics treatment (P < 0.05); the levels of IL-6, IL-17, and TNF-α in acute brucellosis group were significantly increased compared to those of chronic brucellosis group before antibiotics treatment (P < 0.05). The means of IL-6 [(17.83 ± 2.31), (18.45 ± 2.15), (14.52 ± 2.08) ng/L], IL-17 [(17.58 ± 2.21), (18.56 ± 2.29), (13.69 ± 1.85) ng/L], and TNF-α [(370.73 ± 41.04), (375.46 ± 44.61), (290.19 ± 34.67) ng/L] were significantly different in the acute brucellosis, chronic brucellosis and control groups after antibiotics treatment (F=25.94, 39.96, 38.30, P < 0.01); compared to the control group, the levels of IL-6, IL-17, and TNF-α in acute brucellosis and chronic brucellosis groups were significantly increased after antibiotics treatment (P < 0.05); the levels of IL-6, IL-17, and TNF-α in acute brucellosis group were not significantly different compared to those of the chronic brucellosis group after antibiotics treatment (P > 0.05).@*Conclusions@#This study shows that before antibiotics treatment, the bone resorption-related cytokines IL-6, IL-17, and TNF-α of acute brucellosis are higher than those of chronic brucellosis. Antibiotic treatment can significantly reduce their levels and may affect bone resorption in patients with brucellosis.

ABSTRACT

Photodynamic therapy (PDT) is one of the new approaches for cancer treatment with high efficacy. However, applications of current photosensitizers are restricted to skin and superficial tumor due to poor in vivo targeting ability, poor water solubility and short wavelength excitement, which limits penetration therefore therapeutic depth. Here, a biodegradable polymeric micelle, methoxy poly(ethylene glycol)-polylactide copolymer (mPEG-PDLLA), is employed as drug delivery system to co-encapsulate strong two-photon absorption compound (LTPA) and photosensitizers. This delivery system is designed to target tumor passively, resulting in near infrared light with an approximately 808 nm wavelength becoming able to indirectly excite photosensitizers through fluorescence resonance energy transfer. Tumor cells and microvessels could be damaged by the generated singlet oxygen. The average size of drug loaded micelles was approximately 55 nm and showed a spherical shape. Both compounds could be released simultaneously from micelles under either weak acid and neutral pH conditions. Reactive oxygen species was produced intracellularly during two-photon PDT process and induced cell apoptosis/necrosis, which was quantified by Annexin-V/FITC assays. Time-dependent ex vivo organ distribution and in vivo anticancer efficacy results suggested that the drug carriers could accumulate in tumors and suppress tumor growth by two-photon PDT. All animals experiments were performed in line with national regulations and approved by the Animal Experiments Ethical Committee of College of Pharmaceutical Sciences, Southwest University. In summary, we have employed two-photon PDT for breast cancer treatment successfully in a mouse model and have demonstrated the significance of delivery system in such therapeutics.

ABSTRACT

<p><b>OBJECTIVE</b>To explore clinical effects of Kirschner wire with hole transverse fixation combined with titanium cable purse string suture in treating refractory fracture of patellar comminuted fracture.</p><p><b>METHODS</b>From January 2014 to January 2016, 17 comminuted fracture treated by Kirschner wire with hole transverse fixation combined with titanium cable purse string suture were retrospectively analyzed, including 10 males and 7 females aged from 28 to 67 years old with an average of (48.7±3.2) years old. According to AO/OTA fracture classification, all fractures were type 34-C3.2. Operative time, blood loss, intervals of the first functional exercise postoperatively and complications were observed. Meanwhile, function recovery was evaluated by Böstman scoring at 10 months after operation.</p><p><b>RESULTS</b>All patients were followed up with an average of(14.6±2.1) months ranged from 10 to 24 months. All incisions healed at stage I. Clinical fracture healing time ranged from 11.7 to 16.5 weeks with an average of (12.7±1.7) weeks; operative time ranged from 67 to 95 min with an average of (71.4±11.5) min; blood loss ranged from 41 to 72 ml with an average of (57.0±7.5) ml; the first time of functional exercise ranged from 2 to 5 d with an average of (2.2±1.7) d. No complications including Kirschner pin stabbing the skin, loose or slippery of Kirschner pin, bursitis with pain brought by tip of Kirschner pin, wire escaped and broken occurred. Postoperative Böstman score at 10 months were 28.15±1.74, 12 patients got excellent results, 4 well and 1 lose.</p><p><b>CONCLUSIONS</b>Kirschner wire with hole transverse fixation combined with titanium cable purse string suture in treating refractory fracture of patellar comminuted fracture has advantages of simple operation, good anatomical reduction, stable fixation and less complications, which is worth popularizing.</p>

ABSTRACT

Objective To investigate the different exposure and treatment methods for the chorda tympanic nerve in middle ear surgery, and discuss the surgery techniques and the feasibility of the chorda tympanic nerve protection.Methods From September 2013 to March 2016, 155 cases of middle ear surgeries at Zhujiang hospital were included in this study, including 24 cases of type I tympanoplasty, 6 cases of atticotomy and type I tympanoplasty, 22 cases of atticotomy and type II tympanoplasty, 23 cases of canal-wall-up mastoidectomy and tympanoplasty,74 cases of canal-wall-down mastoidectomy and tympanoplasty, 6 cases of stapedotomy.The conditions of exposure and protection of the chorda tympanic nerve in the operation were compared, and their taste function at 3 days to 1 months postoperatively through questionnaires were evaluated.Results The preservation rate of the chorda tympanic nerve was up to 89.03%(138/155).There were 17 cases of chorda tympanic nerve injuries, of which 15 cases suffered hypogeusia with the rate being 88.2%(15/17).In 126 cases of the complete protection of the chorda tympanic nerve, 13 of them appeared hypogeusia at 10.3% (13/126), but they recovered within 1 months postoperatively.One case of delayed facial paralysis occurred in 16 days postoperatively, and recovered completely after 2 weeks of treatment with glucocorticoids.There was a significant difference in the incidence of postoperative abnormal taste between the complete protection of the chorda tympanic nerve and fracture during operation.Conclusion According to the different position and exposure of chorda tympanic nerve, the individual measures should be taken in middle ear surgery to protect the chorda tympanic nerve.

ABSTRACT

BACKGROUND:Umbilical cord as childbirth waste has wide variety of sources and can be easily obtained, without any ethical and legal restrictions. Therefore, human umbilical cord mesenchymal stem cells break al the restrictions originated from other sources of mesenchymal stem cells. OBJECTIVE:To review the application of human umbilical cord mesenchymal stem cells in cartilage diseases, neuroglioma, ischemic brain injury, lung disease, liver disease and models of myocardial infarction. METHODS:The PubMed and Wanfang databases were searched by the first author using the keywords of“human umbilical cord mesenchymal stem cells, disease models, celltherapy”in English and Chinese, respectively. Seventy-three articles were searched and final y, 35 articles were included in result analysis. RESULTS AND CONCLUSION:Human umbilical cord mesenchymal stem cells have multilineage differentiation capacity similar to bone marrow mesenchymal stem cells. Compared with bone marrow mesenchymal stem cells, human umbilical cord mesenchymal stem cells have lower immunogenicity. Human umbilical cord mesenchymal stem cells show certain curative effects on cartilage disease, neuroglioma, ischemic brain injury, lung disease, liver disease and myocardial infarction, indicating that human umbilical cord mesenchymal stem cells can be used for celltransplantation to treat various diseases.

ABSTRACT

BACKGROUND:Reprogramming somatic cells to generate pluripotent stem cells has a wide application in biomedical research. OBJECTIVE:To analyze the effect of different molecular weight proteins in chicken egg-white extract to elevate expression of pluripotent genes Oct-3/4 and Nanog in 293T cells. METHODS:The extracts of chicken egg-white were separated into more than 3 ku and less than 3 ku ingredients to be used for co-culture with 293T cells. There were four groups, 1×105 293T cells per wel , total 500μL. In the control group, 500μL culture medium was added;in the other three groups, 500μL chicken egg-white extract, more than 3 ku and less than 3 ku ingredients were respectively added. Quantitative PCR was used to determine the relative expression levels of pluripotent genes Nanog and Oct-3/4 in 293T cells. RESULTS AND CONCLUSION:By using co-culture method, more than 3 ku ingredients have a role to increase the expression of pluripotent genes Oct-3/4 and Nanog, but less than 3 ku ingredients cannot elevate the expression of pluripotent genes. This indicates that the ingredient of chicken egg-white extract to elevate the expression of pluripotent genes is more than 3 ku proteins.

ABSTRACT

OBJECTIVE@#To investigate the effect of different malleus treatments on the postoperative efficacy in the tympanosclerosis patients receiving ossicular chain reconstruction.@*METHOD@#Fifty-nine patients (62 ears) with tympanosclerosis were treated by ossicular chain reconstruction. All the patients were divided into three groups, including malleus removal group (A, 24 ears), retaining only the malleus handle group (B, 18 ears) and the intact malleus group (C, 20 ears). All the patients were followed up 3 months pre-operation, 3 months and 1 year post-operation by audiometric measurement (the average hearing threshold at 0.5, 1.0, 2.0 kHz HI). Tympanic membrane was examined by ear endoscope.@*RESULT@#The pre-operation mean air bone gap (ABG) in these groups were 40.07 +/- 77.56 dB, 37.31 +/- 76.45 dB, and 36.75 +/- 76.72 dB, among which the difference had no statistical significance (P > 0.05). At 3 months after operation, the ABG in all cases was improved at 0.5, 1 and 2 kHz. The difference of ABG improvement among these three groups had no statistical significance (P > 0.05). One year after surgery, the ABG of the three groups were decreased by 17.92 +/- 9.28 dB, 16.76 +/- 5.19 dB and 10.58 +/- 7.38 dB respectively. The hearing improvement in group C is less than the other two groups (P = 0.03, P = 0.016). The difference of hearing improvement between group A and group B had no statistical significance(P > 0.05). Group A and group B each have one case of tympanic membrane perforation and artificial ossicle falling off.@*CONCLUSION@#The operating processes of malleus in ossicular chain reconstruction of patients with tympanosclerosis were introduced. In terms of short-term efficacy, the three groups showed no significant difference. However, the long-term efficacy of the patients in the group A and group B were better compared with the group C.

ABSTRACT

OBJECTIVE@#To investigate the related parameters of temporal bone structure in the surgery of cochlear implantation through facial recess approach so as to offer a theoretical reference for the avoidance of facial nerve injury and the accurate localization.@*METHOD@#In a surgical simulation experiment, twenty human temporal bones were studied. The correlation parameters were measured under surgical microscope.@*RESULT@#Distance between suprameatal spine and short process of incus was (12.44 +/- 0.51) mm. Width from crotch of chorda tympani nerve to stylomastoid foramen was (2.67 +/- 0.51) mm. Distance between short process of incus and crotch of chorda tympani nerve was (15.22 +/- 0.83) mm. The location of maximal width of the facial recess into short process of incus, crotch of chorda tympani nerve were (6.28 +/- 0.41) mm, (9.81 +/- 0.71) mm, respectively. The maximal width of the facial recess was (2.73 +/- 0.20) mm. The value at level of stapes and round window were (2.48 +/- 0.20 mm) and (2.24 +/- 0.18) mm, respectively. Distance between pyramidalis eminence and anterior round window was (2.22 +/- 0.21) mm. Width from stapes to underneath round window was (2.16 +/- 0.14) mm.@*CONCLUSION@#These parameters provide a reference value to determine the position of cochlear inserting the electrode array into the scale tympani and opening facial recess firstly to avoid potential damage to facial nerve in surgery.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the related parameters of the temporal bone structure for determining the position of implanting electrode into the scala tympani in cochlear implantation surgery through the facial recess and epitympanum approach.</p><p><b>METHODS</b>In a surgical simulation experiment, 20 human temporal bones were studied and measured to determine the related parameters of the temporal bone structure.</p><p><b>RESULTS</b>The distance 5.91∓0.29 mm between the short process of the incus and the round window niche, 2.11∓0.18 mm between the stapes and the round window niche, 6.70∓0.19 mm between the facial nerve in the perpendicular paragraph and the round window niche, 2.22∓0.21 mm from the pyramidal eminence to the round window, and 2.16∓0.14 mm between the stapes and the round window. The minimal distance between the implanting electrode and the vestibular window was 2.12∓0.19 mm. The distance between the cochleariform process and the round window niche was 3.79∓0.17 mm. The position of the cochlear electrode array insertion into the second cochlear turn was 2.25∓0.13 mm under the stapes. The location of the cochlear electrode array insertion into the second cochlear turn was 2.28∓0.20 mm inferior to the pyramidal eminence.</p><p><b>CONCLUSION</b>These parameters provide a reference value to determine the different positions of cochlear electrode array insertion into the scale tympani in different patients.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To describe the incidence and mortality rates of esophageal cancer from 1974-2002 in Cixian county of Hebei province. Basic information on comparative geographical, epidemiological, and clinical research was collected.</p><p><b>METHODS</b>In early 1970s, cancer registry system in Cixian was established, collecting information on all the esophageal cancer cases in Cixian. Data was checked manually, then computerized, coded and analyzed using the software--SPSS 11.5.</p><p><b>RESULTS</b>From 1974 to 2002, there were 18 471 esophageal cancer cases in Cixian, with 11 068 males and 7403 females, respectively. The age standardized incidence rate (ASR) for males was 208.77 per 100,000, while 120.47 per 100,000 for females. The trend of incidence rate of esophageal cancer had decreased during the 29 years from 1974 to 2002 (trend chi(2) = 19.94, P < 0.001). From 25 years of age onward, the incidence rates of the lower age groups declined with the increase of age. As for geographic distribution, the incidence rate in mountainous areas and hilly areas showed a significant declining trend in mountainous areas, chi(2) = 195.00, P < 0.001; hilly areas, chi(2) = 46.08, P < 0.001. The esophageal cancer incidence in plain areas remained steady, but had a slight increase in recent years. From 1969 to 2002, there were 18,736 cases died of esophageal cancer with 11 598 males and 7138 females. The ASR for male was 127.17 per 100,000 and 101.57 per 100,000 for female. Compared with the year 1969, the mortality rate of esophageal cancer in 2002 had a 37.96% decline. The proportion of esophageal cancer among malignant tumors in different decades decreased significantly.</p><p><b>CONCLUSION</b>The trend of the incidence rate of esophageal cancer had been decreasing for the last 29 years. The incidence rate in mountainous areas and hilly areas showed a declining trend while in the plain areas it remained steady but having slight increase in the recent years. The mortality rate of esophageal cancer had a significant decrease from 1969 to 2002.</p>

BACKGROUND: Cixian county has one of the highest incidence rates of esophageal cancer (EC) in China, as well as the world. In 1974, the Cixian population-based cancer registry system was established, so that there is now information on esophageal cancer cases over almost 30 years. METHODS: Data from Cixian Cancer Registry were checked and analyzed using SPSS 11.5. RESULTS: From 1974 to 2002, a total of 18,471 new esophageal cancer cases were registered in Cixian, 11,068 in males and 7,403 in females. The age standardized incidence rate (ASR) for males was 208.77 per 100,000, while for females it was 120.47 per 100,000. There was a clear trend for decrease overall in the incidence rate of esophageal cancer over the 29 years (X(2)=19.94, P<0.001). As to the geographic distribution, the incidence rate in mountainous and hilly areas showed a significant decline (X(2) = 195.00 and X(2) = 46.08, respectively, both P X(2)0.001). Data for esophageal cancer incidence in level land areas in contrast were relatively steadily, with increase in recent years. CONCLUSION: Esophageal cancer has decreased in Cixian county during the last 29 years, but this is due to change in mountainous and hilly areas. Compared to other regions in the world, Cixian county still has a very high incidence of ECs.
